Immunization Requirements
By law, the district must have an immunization record for each student on file. These records should be up-to-date at the time of registration. Records must show the month, day and year of each dose of vaccine and must be validated by a physician’s signature or a public health clinic card. Children entering Garland ISD for the first time must present written verification of the required immunizations at the time of enrollment.
Required immunizations are:
- Diptheria-Tetanus-Pertussis (DTP/DTaP/DT/Td/TdaP)
Minimum of four (4) doses for ages 15 months through four (4) years. Minimum of four (4) doses for ages five (5) and six (6) years with at least one dose since 4th birthday. Minimum of three (3) doses for ages seven (7) years and older with one dose since 4th birthday and within 10 years. - Polio (IPV, OPV)
Minimum of three (3) doses with at least one (1) dose received since 4th birthday. Not required for persons 18 years of age or older. - Measles (Red, Rubeola)
Two (2) doses of vaccine on or after first birthday. The two (2) doses of vaccine must have been received at least 28 days apart. - Rubella (3-day German Measles)
One (1) dose of vaccine on or after first birthday. - Mumps
One (1) dose of vaccine on or after first birthday. - Haemophilus Influenzae Type B
One (1) dose of vaccine on or after 15 months of age or three (3) doses prior to 15 months of age through age 4. History of disease after second birthday must be verified by a signed physician’s statement. - Hepatitis B
Three (3) doses of vaccine for students in grades PPCD-12. - Chicken Pox (Varicella)
One (1) dose of vaccine on or after first birthday for grades PPCD-12. If vaccine received on or after 13th birthday, two (2) doses of vaccine are required and must be at least 28 days apart. History of disease accepted with signed physician statement or signed parent validation form. - Hepatitis A
Two (2) doses of vaccine on or after first birthday and six (6) months apart for students in grades EC, PPCD, PK. - Pneumococcal (Prevnar, PCV7)
Four (4) doses of vaccine in infancy with one (1) dose after first birthday, two (2) doses of vaccine on or after first birthday, or one (1) dose of vaccine on or after second birthday for students in grades EC, PPCD, PK.

Immunization records
To allow student immunization records to be released from Garland Health Dept to GISD Health Services, the Release of Immunization Form must be on file with the Garland Health Dept.
